23 year old male has severe face and arm muscle spasms and can hardly speak. Patient is infected with Clostridium tetani from a dirty needle at the tattoo shop.

Systems Affected
Muscular System
Severe muscle spasms from the bacteria, causing continued contractions which can lead to muscle fatigue
Integumentary System
Skin directly impacted from the tattoo needle & propelling bacteria further into his skin near blood vessels, allowing bacteria to spread
Nervous System
Action potential events at NMJ are continuing to fire, causing the overwhelming amount of muscle spasms

Respiratory System
Severe spasms can spread to both the internal airway & diaphragm, causing insufficient oxygen supply & death
Steps of Exocytosis

- Vesicle nears plasma membrane
- Vesicle membrane fuses w/ plasma membrane
- Plasma membrane opens to outside of cell
- Release of vesicle contents into the interstitial fluid & integration of vesicle membrane components into the plasma membrane
Phospholipid molecules of vesicle & plasma membrane bilayers rearrange; requires cell to expend energy by splitting ATP
Differences Between a Twitch, Summation, & Tetany
Twitch: single, brief contraction period of a skeletal muscle in response to a single stimulation
Voltage is increased in increments until a muscle responds, mainly producing a twitch; minimum voltage needed to generate twitch is the threshold

Contraction Period: repetitive power stroke pulls on thin filaments, shortens sarcomeres; muscle tension increasing

Patient is experiencing spasms instead of flaccid paralysis or tetany since the neurotoxin tetanospasmin has blocked the inhibitory function of the neurons, preventing the release of neurotransmitters; therefore, repeated spasms are taking place
Patient is also in the early stages of the tetanus infection, therefore, it has not completely reached his bloodstream where the effects of the spasms can result in muscle fatigue and later, complete tetany
Patient received tattoo with an unclean, probably rusty needle, which has cause him to be stabbed multiple times over a course of hours with the bacterium
