CNSLF Path MS
Intro
-
immune mediated inflamm disease of CNS characterised by demyelination (destruction of normal myelin)
-
-
classically thought to affect white matter (axons), now also known to affect grey matter (neurons)
-
-
Myelin wraps around (insulates) axons in concentric rings - ensures rapid transmission of AP along axon

-
Dx
-
McDonald criteria
objective demonstration of dissemination of CNS lesions in both time (2+ attacks) + space (2+ lesions), based on clinical findings alone or a combo of clinical + MRI findings (different sites @ different times)
-
-
-
-
Acute tx
-
plasma exchange may be beneficial on those not responding to glucocorticoid tx with a severe relapse
other input may be needed - physio, OT, SALT, ophthalmology etc
-
-
-
Pathogenesis
active MS plaque
can affect brainstem, cerebellum, cord, cerebral hemispheres
