Assessment
The School Function Assessment would be best utilized in the school setting with Sophie. It is a criterion referenced, 5-10 minute assessment that will tell us what her strengths are and what areas she needs to improve on to increase her level of function and overall independence in the school setting.
Interventions
Sophie will be provided with an adapted writing utensil to help improve her grasp and increase control while writing.
If Sophie is not successful with the adapted writing utensil, modifications will be made to the way she completes her written work. Sophie will be given skilled instruction on how to use a tablet or computer to type her written work to increase her independence with written expression.
Sophie will also be instructed in how to use Snap Type app on ipad to allow her to complete worksheets requiring written answers to increase her independence with school work.
If Sophie continues to have difficulties after adapting the writing utensils, she can then practice writing on top of her slant board to facilitate better positioning of her wrist during writing tasks.
Sophie will be provided with utensils with built-up handles to decrease her difficulties with sustaining a grasp of her utensils at lunch time.
If Sophie is unable to demonstrate increased independence with using utensils with built-up handles during lunch a universal cuff will be provided to help her maintain her grasp of the utensils.
Sophie's occupational therapist will collaborate with her PE teacher about making a modification to the scarf juggling activity. Sophie can tap/hit a balloon back and forth with another student to work on the same visual motor and eye-hand coordination skills without requiring her to grasp it.
If using a balloon is too easy for Sophie, a beach ball can be used instead, which would require her to use her both upper extremities together to catch the ball, but does not require more skilled grasping techniques of her hands.
Sophie can be provided with a slant board to use at her desk when reading books to help improve her overall posture and positioning to help her increase her attention to reading activities.
If Sophie is continuing to struggle to participate independently in reading tasks due to decreased positioning and difficulty manipulating the materials, she will be provided with a rubber finger tip grip to help assist her with turning the pages of the book, so she does not fall behind due to her inability to independently turn pages.

Occupational Profile
Sophie receives speech, OT, PT, and adaptive PE services at school.
Sophie is eligible to receive special services under the categories of Orthopedic Impairment; Speech Impairment, Specific Learning Disability. She has difficulties with reading comprehension, math reasoning, and written expression.
