Ovarian Cancer
Overview
- Leading cause of death from gynae cancer
- 5th most common chance win women
- 2% lifetime risk in Eng & Wales
- 5YS < 35% due to presenting with advanced disease
- Stage most important for outcome
Epithelial ovarian cancer most common, but Fallopian tube and primary peritoneal cancer treated as the same entity
-
- Completeness of surgical resection is a major prognostic factor
- Most women develop recurrent disease, maybe due to incomplete removal of tumour - 'complete' is assessed by surgeon not by histopathology from surgical specimens at margins
-
Very important to determine whether resection is possible or whether neoadjuvant chemo needed first - incomplete debunking carries risk of morbidity even aside from leaving remaining tumour
-
NICE guidelines
-
Stage 1 cancer perform surgical staging (oophorectomy, LN assessment and removal) and adjuvant chemo only if high risk stage 1 disease
