Acute severe: Fuclox + gent IV
Subacute: Benpen + gent IV
By organism
Streps: benpen + gent IV
Enterococci: amoxicillin + gent IV
Staphs: fluclox ± rifampicin IV
Fungi: flucytosine IV + fluconazole PO.
Amphotericin if flucytosine resistance or Aspergillus
Skin
Erysipelas
Pen V 500mg QDS or ben pen
Impetigo
Localised: topical fusidate
Widespread: fluclox 250 QDS
Cellulitis
Empiric: fluclox 500mg QDS
Known Strep: Pen V or Ben pen
Sepsis
Tazocin
± Vanc if MRI suspected
± Gent for gram -ves
If anaerobe: cef and met
GIT
C. diff
1st: metronidazole PO
2nd: Vancomycin PO
GI sepsis: cefuroxime + metronidazole
Campylobacter: ciprofloxacin
Shigella: ciprofloxacin
Meningitis
Immediate
Community: benpen 1.2g IV/IM
Give Dex 0.15mg/kg/6h with first dose of Abx unless in septic shock or meningococcal sepsis
Bacterial
under age 50: ceftriaxone 2g IVI/IM BD
over age 50: ceftriaxone + ampicillin 2g IVI /4h
Viral
aciclovir
Genital Tract
Chlamydia / NSU: azithromycin 1g STAT
Gonorrhoea Azithromycin 1g STAT + ceftriaxone 500mg IM
PID: ofloxacin + metronidazole
Urinary Tract Infection
Prostatitis: Cipro 500 mg BD for 28d
Pyelonephritis: cefotaxime 1g IV BD for 10d
Lower UTI: Rx for 7d
1st
Trimethoprim 200mg BD
Nitrofurantoin 50mg QDS (avoid in ↓ eGFR)
2nd
Cephalexin 500mg BD
Amoxicillin 500mg TDS
