Stromal tumours
fibroadenoma
most common benign tumour of female breast
occur after puberty, usually before 30
usually solitary (20% multiple) well-defined freely moveable mass ('breast mouse')
combined prolif of epithelium + connective tissue - fibrous stroma compressing glands
respond to oestrogen + progesterone - size may fluctuate with menstrual cycle
pregnancy may cause increased growth
regress after menopause
phyllodes tumour
larger + older (>45) compared to fibroadenomas
morphologically resembles fibroadenoma but may grow to large size (1-15cm in diameter) - thus aka giant fibroadenomas
'leaf-like': clefts + slits - thud aka cystosarcoma phyllodes
combined prolif of epithelium + connective tissue (predominantly stroma)
can be benign or malignant
the malignant variant exhibits increased stromal cellularity, pleomorphism, increased mitotic rate + infiltrative growth pattern
malignant variants tend to recur locally but may met (15% of cases)
