A skinny, 22 year old has severe cramping in calves, thighs, buttocks, back and shoulders, and increased fatigue. Patient is involved in excess weight training and cardiovascular training. Patient sees no results and has a poor diet which has resulted in skipping meals.
Upstream Causes
Direct - The direct cause of patients inability to gain muscle excess fatigue and cramping is due to the wrong training needed to achieve muscle gain and insufficient nutrients and proteins involved in the poor dieting
Indirect - Poor eating habits , skipping meals , insufficient research into workout regimen.

3 Types of Muscle Tissue
Skeletal Muscle - Organs that attach to and cover the skeleton. Long muscle fibers and striations. Voluntary control. Responsible for overall body mobility. Activated by reflexes.
Cardiac Muscle - occurs only in heart. striated and involuntary. contracts without stimuli of nervous system. Contracts at a fairly steady rate set by the hearts pacemakers
Smooth Muscle - found in walls of visceral organs , such as stomach, urinary bladder, and respiratory passages. forces fluids through internal body channels. dilates and constricts pupils. Long elongated cells , no striation, involuntary.

Skeletal muscle is made of muscle fibers , nerves, blood vessels, and connective tissue
Nerve and blood supply- one nerve, one artery , and one or more veins serve each muscle. every skeletal muscle fiber is supplied with a nerve ending that controls its activity.
Skeletal muscle has a rich blood supply. Contracting fibers use huge amounts of energy and require almost continuous delivery of oxygen and nutrients via arteries
Capillaries flow through muscles and straighten when the muscle stretches and contort when the muscle contracts.
Connective tissue sheaths - in tact muscle is composed of connective sheaths that support each cell and reinforce and hold together the muscle, preventing the bulging muscles from bursting during exceptionally strong contractions.

Perimysium and fascicles - within each skeletal muscle , the muscle fibers are grouped into fascicles that resembles bundles of sticks. surrounding each fascicle is a layer of dense irregular connective tissues called perimysium
Endomysium - "within the muscle" is a wispy sheath of connective tissue that surrounds each individual muscle fiber. it consist of fine areolar connective tissue.
Muscle fiber - an elongated multinucleate cell , branded striated appearance.
Myofibril - rodlike contractile elements that occupy most of the muscle cell volume . composed of sarcomeres arranged end to end, appear banded.
Sarcomere - is a contractile unit , composed of myofilaments made up of contractile proteins .
Myofilament - contractile myofilaments of two types. Thick filaments contain bundled myosin molecules ; thin filaments that contain actin molecules. These provide elastic recoil the tension is released.

A single muscle cell is composed of hundreds and thousands of rod like myofibrils that run parallel to its length
myofibrils are made of a chain of sarcomeres linked end to end. and contain even smaller rodlike structures called myofilaments

Muscle Fatigue
Vigorous muscle activity cannot continue indefinitely. Muscle Fatigue is a state of physiological inability to contract even though the muscle is still receiving stimuli.
ATP levels inside muscle cells do drop, but muscle fatigue serves to prevent complete depletion of ATP in muscle, which would result in death of muscle cells and rigor mortis.

Downstream Effects

Effect 2 - because patient wants to gain muscle the complete opposite will happen due to the wrong workout routine
Patient will continue to lose muscle instead of gaining due to the excess running and lack of nutrient intake which muscles need in order to build, this will result in weak skeletal muscle and increase fatigue
Effect 3 - Skipping meals is very unhealthy and will not contribute to patients goals in achieving muscle mass
lack of nutrient intake will result is tiredness, headaches, lower metabolism levels, and increase diabetes.
Effect 4 - lack of sugar intake can cause lightheadedness, fatigue, unresponsiveness.
