A hip fracture is characterized by a break in the proximal end of the femur. Surgical fixation is needed for correct healing.
Acute pain r/t fractured right hip and right hip fixation surgery AEB pain rating 8/10 unresponsive to pain medication.
Interventions: Maintain proper positioning of operated extremity, provide comfort measures and diversional activities, provide medication on regular schedule, rate pain on a scale of 0-10, apply ice pack q4hrs per providers orders.
Goal: patient will demonstrate an understanding of correct positioning of RLE, patient will report a decrease in pain; patient's tolerable pain goal is 2/10.
Risk of infection r/t impaired skin integrity of RLE Interventions: wash hands and wear gloves when assessing incisional sites, monitor sites for signs if infection, intsruct patient not to touch surgical site, investigate reports of increased incisional pain or changes in the characteristics of pain, monitor temperature and encourage fluid intake.Goal: patient will remain free of infection, patient will verbalize and report signs of infection.
