Daily Positive Behavior Tracking Form
The daily positive behavior tracking form seems to align perfect with my intervention that involves talking to the parent. This form allows for the students behavior to be tracked throughout the day and it can be reported back to the parent everyday. It also provides the ability to keep track of how the student responds to non verbal cues, and taking a break, by keeping track of each intervention when its used on the form and documenting how the student acts after these interventions are used and provide data to review.

Intervention General Tracking Form.
For Tier 1 interventions I will keep track of the behavior through a general intervention tracking form. This will allow for me to keep track of what is working and what is not. As well as provide a template for moving forward into tiers 2 and 3. We can keep data on the interventions used and have some reflection back on what we should try new or keep trying.

For Tier 2 data tracking I have decided that a daily positive behavior tracking form would work good once again. This allows for the student to keep track of when they are being good and when they are not following school expectations. This student reacts well to positive praise and is very good at taking constructive feedback so I believe that allowing him to keep track of his own behavior will provide an opportunity for the student to see the behavior and reflect on where it needs to improve at.

At this point, it would appear that the self evaluation is not working for the student. While we continue with interventions that use positive praising and allow for self reflection by the student on their behavior. It will now be time for behavior consequences. This is another opportunity for the student to keep track of his behavior and be able to really reflect on how the behavior has effected school and learning. By keeping track of everytime the student gets in trouble and reviewing all the consequences that come from getting in trouble due to behavior. Hopefully this will allow the student to reflect and continue to build on their daily behavior.

Student Progress Self-Evaluation
For Tier 2 interventions I have decided to use the student progress self evaluation worksheet. This will allow for some self reflect and allow the student to critique themselves and decide if they are truly putting in the effort they should be putting into their work. Students in 7th grade seem to do well in self reflection and are at the age where they can take responsibility for their actions. This form allows for the student to do so, and can even see how their grades are being effected by the lack of effort being put in on a daily basis.

School Counseling Referral Form.
At this point I would go with a school counseling referral form. At this point it seems the student may be choosing to not do the work for different reasons, that might have to do with more than paying attention. Suggestions this data tracking form will allow someone with a professional background to evaluate and report on the student and the areas they struggle in within school. As well as provide an opportunity to talk to the student and really see what is wrong, whether it be a learning disorder or adhd etc.
