ACCIDENT AT WORKPLACE
PREVENTION
- Heed warning and safety notices posted on the machine and in the area
- make sure that footwalks, handrails, barriers, guards, and safety fences/gates are in place and in good working order before starting the machine
- Never thread closed nips by hand. Refer to the OEM or mill threading instructions.
- All roll cleaning equipment must have long handles to keep the user's hands and arms out of nip areas.
- Keep all parts of the body away from belt drives
- Do not remove broke by passing it through calender roll nips. Hands and/or body parts can become entangled in the broke and be pulled through the calender nip
ACCIDENT INVESTIGATION
- Preserve/ Document the Scene
- incident date and name of investigation personnel documented
- Injured employees name, injury description (permanent/ temporary), date and location of accident, pictures/videos of incident
- Collect information
- collected via interviews ,document reviews.
- the injured worker is interviewed to ensure all information is pertinent to the incident

- Implament corrective actions
-Should entail program level improvement and should be supported by senior management
-The finding of investigation will shape perceptions and subsequent corrective action
