Slough/ low-moderate exudate
Aim of products- Promote autolytic debridement, tissue viability, and a moist healing environment as well as moisture balance. Products should protect the periwound by absorbing exudate; preventing inflammation and infection.
Mesalt daily or every 2 days for 5 days
If wound does not have the capacity to heal use a combine or Gamgee
If the wound has the capacity to heal use vacated and a non adherent dressing
Aim of tx- remove slough and provide a clean wound bed for granulation to take place
Clinically colonised and clinically infested wounds
Aim of tx- Remove microbial load and bacterial bioburden (the amount of bacteria on the skin, a high amount can lead to poor wound healing).
Aim of products- provide moisture balance as well as providing antimicrobial cover.
Aquacel ag (up to 2 weeks then review)
Malodorous/ fundating wound- high exudate
Aim of tx- Manage odour, absorb exudate that may cause the smell, provide comfort.
Aim of products- same as am of tx
For wounds that have the capacity to heal- mepilex ag with border, mesorb, mextra
For wounds that do not have the capacity to heal/ palliation- combine, Gamgee, use of odour diffusers and kitty litter, activated charcoal dressings
Dry/ fragile peri wound
Aim of tx- prevent tissue damage, restore skin integrity.
Aim of products- Rehydrate and sooth iritated skin and provide a protective barrier.
Soft dressings that minimise the risk of further irritation to the skin
Painful wounds and tissue trauma at the time of tissue change
Aim of tx- reduce pain, reduce and treat the tissue trauma
Aim of products being used- moisturising irritated skin, adherence of product
Silicone atraumatic dressings
medicated gels such as morphine and lignocaine
soft silicone pads
