Temporomandibular Dysfunction - Clicks & Locks
Closed Lock
The condyle never assume a normal relation to the disc
The disc moves ahead of the condyle
the condyles forward translation is restricted by the disc
Single Click
Occurs between positions 2 and 3
normal function occurs in the remaining opening and closing movement
the click is felt as the condyle moves across the posterior border into the intermediate zone of the disc
in the closed position the disc is displaced forward and medially by the superior lateral pterygoid
Reciprocal Click
Clicking occurs in
both
directions
Click 1
occurs between positions 2 & 3, and is felt as the condyle moves across the posterior border of the disc
Click 2
occurs between positions 8 & 1, and is heard as the condyle once again moves from the intermediate zone to the posterior border of the disc
Open Lock
there are two clicks in opening
the condyle moves over the posterior rim of the disc
when it moves over the anterior rim of the disc
there are two click on closing
same as opening just reverse order
if the disc lies posterior to the condyle on opening after the second click it may not allow the condyle to slide back
if the condyle dislocates outside the fossa, it is a true dislocation with open lock
Functional Dislocation with Reduction
Subluxation or dislocation of the disc may occur, which the condyle must override the posterior rim of the disc later than normal during mouth opening
During opening, the condyle passes over the posterior border of the disc into the intermediate are of the disc --> reducing the dislocated disc

occurs between positions 3 &4
Click 2
occurs between positions 8 & 1
Bruxism
forced clenching and grinding of the teeth, especially during sleep
may lead to facial or jaw pain
Normal Functional Movement
during opening the disc is rotated posteriorly on the condyle as the condyle is translated ou of the fossa
closing is the exact opposite of the opening movement
