Doctor may also order ultrasound to look for cyst, tumours and other obstructions within the urinary tract
Doctor will check for fever, tenderness in the abdomen and othe common symptoms if suspect kidney infection

Risk factors
Any problem that interrupt the normal flow of urine causes greater risk of acute pyelonephritis for example urinary that has an unusual size and shape is more likely to cause acute pyelonephritis

Treatment
Initial treatment of women with acute pyelonephritis who require hospitalization should include an intravenous antimicrobial regimen, such as a fluoroquinolone, an aminoglycoside with or without ampicillin, an extended-spectrum cephalosporin or penicillin with or without an aminoglycoside, or a carbapenem
Treatment options for women with acute pyelonephritis not requiring hospitalization include 500 mg of oral ciprofloxacin (Cipro) twice per day for seven days; 1,000 mg of extended-release ciprofloxacin once per day for seven days; or 750 mg of levofloxacin (Levaquin) once per day for five days. These options are appropriate in areas where the prevalence of resistance to fluoroquinolones does not exceed 10 percent.

Causes
The infection usually starts in the lower urinary tract as a urinary tract infection. Bacteria enter the body through urethra and begin to multiply and spread up to the bladder from there the bacteria travel through the ureters to the kidneys, bacteria such as E. Coli often cause the infection but any serious infection in the bloodstream can also spread to the kidneys and cause acute pyelonephritis
Clinical manifestation
A person may experience pain in this areas, back, can occur during urination, in the flank, or pelvis
Urinary: blood in urine, cloudy urine, foul smelling urine,
frequent urge to urinate, frequent urination, or urinary tract infection
Whole body: chills, fatigue, fever, loss of appetite, or malaise
