Kaitlyn Hunter (names have been changed because this is actually one of my clients)
Tier 1
Kaitlyn needs consequences for not completing her assignments but she also need reflection on why it is important that she maintains her grades and completes her tasks at school. She is old enough to understand that there are consequences for her behavior that could possibly result in her future being negatively influenced. Kaitlyn could benefit from a bit more structure to help keep her focused on her tasks and not allow her mind to roam and think about her depressive state. School needs to be an outlet for her. She needs to be taught techniques on how to be academically successful while understanding the importance of socializing and identifying her mood. When Kaitlyn has a good day a note needs to be sent home detailing this so she could receive some rewards but when she has not been focused at school a note needs to be sent home detailing that as well in order for her parents to create consequences that are logical and natural.

Tier 2
Behavior Contract
Behavior Intervention Plan (BIP)
Kaitlyn needs to have her teachers as well as her therapist meet for an IEP and as well as a behavorial intervention plan. This plan will allow her to see the consequences of her actions as well as give her an opportunity to self correct before administering consequences. Kaitlyn is old enough to be responsible for how she deals with conflict. She needs to learn resolutions whether it is resolving with family, friends, teachers etc. She needs to again be held accountable for her actions and responsible for completing her assignments. During this session, her depression needs to be discussed as well as a plan moving forward. If Kaitlyn is on medication then it is important that at this meeting that information is given to teachers and faculty.
Tier 3
Kaitlyn needs coping skills and need to be taught about relationships (professional and personal). In order for Kaitlyn to socialize she needs to learn how and the importance of socializing with others. She can also benefit from being praised for applying the skills and techniques. Since Kaitlyn explained that she is depressed and stressed out at times it is critical that she learns relaxation techniques that will allow her to stay focused when she is at school.
