Talk to student, ask why.
are they having trouble understanding the material?
are they being bullied?
Many times bullies will act only when they know they wont be caught.
Are they bullying others?
Is the material too easy?
Are they bored?
If appropriate, talk to admin about moving the student to a higher level
Not talking in class
Talk one on one (quiet students sometimes are embarrassed to speak in-front of their peers.)
are they shy?
If just shy, try to accommodate them while encouraging them to speak more in class.

Student is having problems completing assignments
Informed by administration
Discuss with admin the issue and what resources/accommodations the student needs
Discuss with the student the issues and how they want to do in class
Continue discussing how best to implement resources in class
Discuss with parents if necessary
Student admits having problems
Discuss with student the nature of the problems
Do they have an IEP/504 plan?
If yes talk to administration and see about how best to implement in class
If no talk to them seriously about getting extra help.
Bring in admin, and potentially start discussing with parents
Did they just miss one assignment?
Are they struggling with just one topic?
The student may just need a little of help on this topic
if not appropriate, find extra ways to keep the student engaged in class.
