Imbalanced nutrition
Diarrhoea related to the abdominal cramping
Anxiety related to knowledge deficiency about the disease
EVALUATION
Normal vital signs
Inflammation monitored
The patient to report that pain is controlled
Correct consistency of the normal stool should be reported by the patient
CARE PLAN OF INTESTINAL OBSTRUCTION
Defination
This is gastro intestinal condition in which digested material is prevented from passing normally through bowel.
NURSING INTERVENTION
Provide mouth care to relieve nausea
prevent vomiting
Maintain normal vital signs
monitor weight
Monitor patients pain level
EXPCTED OUTCOME
• No pain
• No nausea and vomiting
• Maintain normal hydration status
NURSING DIAGNOSIS
Pain due to the stretched intestinal muscles
Nausea and vomiting that may be projectile and contain bile
Severe constipation related to no stool passed
Abdominal distension due to accumulation of intestinal contents
Dehydration
High temperature evidenced by intense thirst, drowsiness
EVALUATION
Pain controlled
Nausea and vomiting stopped
Restored normal fluid and electrolytes
Relieved obstruction and returned to normal bowel function

Defination
A collection of pus in the tissue around the anus and rectum
NURSING DIAGNOSIS
Pain in the anus or rectum

Monitor bleeding in the stool
Give adequate amount of fluid
Administer stool softener as prescribed
