ANAPHYLAXIS
Initial management
Remove/discontinue the causative agent
pulse oximeter +/- supplemental oxygen
cardiac monitoring
ABCs +
epinephrine
0.01 mg/kg of 1:1000 SC/IM
Repeat every 5' as needed
IM in the lateral aspect of the thigh
Secondary management
Bronchospasms?
Albuterol/Salbutamol MDI
mild-moderate symptoms - 4 to 8 puffs
severe symptoms - continuous nebulization (0.5 mg/kg/hour) to a maximum dose of 20 mg/hour
IV access
Diphenhydramine 1-2 mg/kg
Ranitidine
Methylprednisolone 2mg/kg IV bolus
Hypotension
20 mL/kg fluid bolus NS

Rapid onset IgE mediated (type I) hypersensitivity reaction involving multiple organs
Cutaneous/mucosal (flushing, urticaria, angioedema)
Respiratory (laryngeal edema, bronchospasm)
GI (vomiting, diarrhea, nausea)
Circulatory (hypotension, tachycardia)
