Identify
Report observations to homeroom teacher and PYP coordinator to observe and evaluate the student
Teachers hold a meeting with the parents to discuss the evaluative findings. Referrals for diifferentiated learning plans and interventions are discussed and considered with parents, homeroom teacher and specialists.
If parents do not agree with the assessments, other options can be suggested by the school or the parents may seek a second professional opinion.
School provides support and teachers review new assessments and provide recommendations to meet the student's educational needs.
Plans are put into action
Student is regularly monitored and assessed
Student responds to plan and shows signs of progress.
Supportive and reassessment meetings are regularly scheduled involving parents, homeroom teacher and any affected specialist teacher
Student does not show positive responses to the learning plan
School gives documentation on the support given to the student and how it hasn’t been enough. They recommend other special education options that they feel could meet the needs of the student.
School follows up on student's progress and keeps records up to date
