Dilated CM
Path: Chambers dilate, thin walled, floppy.
Causes:
- Viruses
- Wet BeriBeri
- Etoh :beer_mugs:
- Ischemia
- Chemo (CTX)
Pt: Systolic CHF
- Orthopnea
- DOE
- PND
- Crackles
- Peripheral edema
Dx: Echo: see dilation
Tx: Symtom management CHF
- BBlocker & ACEi :spades:
- Diuretics: Loops :roller_coaster:
- Transplant
Etiology specific:
- Alcohol abstinence
- Stop CTX

Restrictive CM
Path: Stuff fills up ventricle wall, no relaxation.
- Amyloid
- Sarcoid
- Hemachromatosis
Pt: Diastolic HF
Dx: Echo = restrictive pattern
- speckled pattern: amyloid
- patchy : Sarcoid
Other s/s to help find etiology
- amyloid = peripheral neuropathy
- sarcoid = Pulmonary disease
- Hemachromatosis = cirrhosis or bronze diabetes
Dx:
- Amyloid: Gingiva or fat pad biopsy
- Sarcoid: Cardiac MRI w/ endomyocaridal biopsy
- hemochromatosis: Ferritin (elevated) & genetic test
Tx: Diastolic CHF protocol: BBlockers :trumpet: = CCB :icecream:
- :warning:gentle diuresis, watch out in RHF, need to be able to fill ventricle
- Transplant
- Goal: treat underlying disease ( usually ineffective, high mortality)
- Generally doesn't matter how you get it the response is the same.
- Anything that causes insult to the myocardium.
