After completion of the form, the SENCO notifies Head of Department of required SEN observations and make recordings.
Not a SEN issue. Student is referred to Head of Department to take measures, such as meeting parents.
SEN Issue – SENCO meets with Head of Department to discuss findings and possible causes.
SENCO and Head of Department meet with parents to discuss issue.
Parents are in denial and unwilling to accept the findings so pupil remains undiagnosed.
SENCO and classroom teachers continue to make observations and recordings as necessary. The teacher(s) differentiate and support as much as they can.
Parents are willing to get the child diagnosed by medical professional.
SENCO follows the recommendation of the physician and creates an IEP.
IEP is discussed with class and support teachers and approved by Head of Department and Principal
