- weight gain (fluid)
- Displaced lateral PMI

Right Heart Failure
- IVC
- SVC
- DOE (due to hypo perfusion)
Left Heart Failure
- IVC
- SVC
- Pulm edema (DOE and crackles and orthopnea and PND)

Etiologies
Systolic dysfunction(HFrEF)
- floppy walls
- poor forward flow (EF<50)
- Ischemia, and many other causes
Diastolic Dysfunction (HFpEF)
- HTN, infiltrative diseases
- beefy muscular heart with low ventricular volume
- high or normal EF
- treatment is not clear, treat the same as HFrEF

Dx: CHF exacerbation suspected
- CXR
- Troponins and EKG
- BNP

CHF specific labs
- BNP
- not specific to heart failure, more a sign of atrial volume overload (ESRD)
- good enough for HF
- 2D echo
- EF
- Pulmonary artery pressure
- Diastolic dysfunction
- Angiogram (L heart cath)
- determine if ischemic or not?
