I am on my Fieldwork IIA, picking up patients for my CI during week 9. I discovered that there is no evaluation in the chart and no notes have been written for 2 months. My supervisor has asked me to “fill in the documentation” so the facility can bill for these treatments. I don’t even know if the treatments were done but this doesn’t seem right to me. What should I do?
Potential ethical issues
My course instructor is not keeping up the AOTA standards, specifically, Standard II - "Screening, Evaluation, and Re-evaluation" (AOTA, 2005).

Veracity
My CI has not been true to themselves as a clinician nor their patients as they have not kept up with their documentation in a timely manner.
Ethical approach to case
I would reach out to my OT coordinator as well as my Academic FieldWork Coordinator for guidance on how to best approach the situation and how to mitigate this in the future.
I would talk to my supervisor ask them how this happened and provide/suggest a tool that I think may help prevent this from occurring again.
Collaborate with the other clinicians on staff; go over the Occupational Therapy Code of Ethics and ensure that we are all on the same page (AOTA, 2015).
Negative solutions
Possible negative outcomes that could arise from this situation are my supervisor reporting that I didn't complete the evaluation in its entirety and report that to my AFWC and give me a negative review on my performance evaluation.

Resolution of scenario
I would not move forward with what my CI is asking of me as I don't feel comfortable doing that; it's going against my ethics as an occupational therapist student and this request of my CI is negatively impacting him/her and their clients' care.
