Service users and their families will be involved in the process to ensure a comprehensive assessment is carried out, a full understanding of needs is gained

Evaluation is carried out by a designated person, a date for the first review will be agreed, this enables su to have a formal evaluation so they can identify problems and have them formally addressed
Professionals and service users and their families examine the aims and objectives for the care plan

Each professional will be fully informed through the care planning process about how they should carry out their responsibilities

Care worker completes a visit they will record it in the care plan therefore implementation is clear

All professionals involved are responsible for checking their aspect of the care plan is working effectively
Care manager will have overall responsibility to ensure that regular checks are being completed and recorded so problems are quickly identified and rectified

Service user and their family will be involved in the monitoring stage and they know if the plan is working effectively
Monitoring can be completed by telephone contact or agreed meetings so helping the service user to have an ease of contact if problems do arise
A care plan will be drawn up addressing the needs of the service users for the return to their own homes
Plan is written down in a format that the service user with a disability will understand which enables them to be clear about what has been agreed and what will happen
Enables good communication between service users, their family and professionals
A review date allowing the service user, family and professionals to check if the plan is working
Problems identified in the evaluation process changes will be made and a new care plan is drawn up - helps to keep the planning process on-going and cyclical

Modifications are written up so that all professionals, carers, the service user and their family are clear and agree the adaptions made by keeping service users informed
