Calcium :glass_of_milk: (Path: (Parathyroid glands: PTH in vesicles (PTH…
Calcium :glass_of_milk:
-
Hypocalcemia (easy)
Pt: s/s tetany, perioral tingling, Trouseau (carpopedal tetany), Chostek's (tapping on face)
Dx: Check Albumin
Corrects Ca?
-
:green_cross:(-)
Ionized Ca normal?
-
:check:(+) normal, bogus lab
-
Etiologies
-
-
Vitamin D deficiency
-
Pt: MILD not deranged, but w/ osteopenia
-
CKD (low yield)
-
early CKD, impairment of VitD formation
-
-
Pancreatitis (low yield)
drop in Ca is part of Ransom criteria, Disorder of inflammation, not Ca
Hypercalcemia
Etiologies:
Hyperparathyroidism
-
Presents: Hypercalcemia
-
brown tumors
:pencil2:not malignancies, but destruction of bone
Dx: elevated PTH
if inappropriate rise in PTH, Kidney wins and PO4 falls
CaSR should turn off, but Parathyroid continues to release independent of Ca blood
-
Determining which type
Sestamibi scan
1º: one gland lights up, other glands atrophied
3º: chronically low Ca (inability to make VitD) leads to upreg of PTH, Constant stimulation of PTH causes multiple secreting adenomas
Tx: same for both, Resection
watch for hypocalcemia, atrophied glands may not come back fast enough
-
-
Hypervitaminosis D
:warning:eating lots of VitD (uncommon, always wrong)
Granulomatous disease:
- Sarcoidosis
- TB
- Histoplasmosis
- Berylliosis
- Leprosy
-
-
-
-
Pt: s/s kidney stones, psychic moans (AMS), abn groans, painful bones
-
-
-
"artificial" 1,25 VitD from granulomas (TB/sarcoid)
-
