negative key features: no syncope, no chest pain, no dyspnea, no body aches or joint pain, no neck stiffness, no AMS or confusion, no diarrhea

Fever, chills, sweats, cough with or without sputum production. Changes in character or color of secretions in patient with chronic cough. Fatigue, headache, tachypnea, tachycardia, crackles or rhonchi heard on auscultation. Signs of infiltrates on imaging
This is my most likely diagnosis. He has possible infiltrates on CXR with productive cough, fever, and leukocytosis.
Irregulary irregular rhythm with no discernable P waves. Tachycardia and palpitations.
Visible on ICU tele monitor. Confirmed on EKG. Felt to be secondary to stress, dehydration, and infection.
Dry mucous membranes, headache, weakness, dizziness, thirst, common. AMS and syncope possible.
Suspected to be a component of current illness. Given 2L NS bolus and encouraged oral fluid intake.
Malaise, fever, chills, body aches, weakness, cough, headache.
Has a nausea, weakness, fever, and headache. Denies body aches, sore throat, or dry cough, or nasal stuffiness. Cough is productive. Will swab to rule this out.
Exposure to hot climate with acute onset. Dry mucous membranes, hyperthermia, headache, weakness, dizziness, thirst, common. AMS and syncope possible with heat stroke.
Was the suspected diagnosis by PCP. However, pt has had ongoing symptoms and fever for 2 days. Would not expect hyperthermia to persist after moving indoors and resting.
Nausea, vomiting abdominal pain, and diarrhea with ingestion of undercooked or contaminated meat or produce or
Pt denies abdominal pain and diarrhea or exposure to undercooked or contaminated foods.
Severe headache, nuchal rigidity, AMS, fever, confusion, agitation, restlessness
No AMS, neck stiffness, confusion, agitation, or restlessness. Doubt meningitis for now.
