Off Task-Non Disruptive
Tier 1 Interventions
Proximity-Have the student sit near the teachers desk. Have their workspace be in an area where there are less distractions. Might be best to have that student sit at a table alone.
Encourage the student to finish their work by having a timer. This will help the student to stay on task. I have seen this work in my classroom for students who are easily distracted.
Explain assignment or activity and have the student repeat back what he/or she will be doing to complete the assignment.
Praise student when they are staying on task. The teacher could even give a reward or incentive to help the student stay on task.
Tier 2 Interventions
Check in Check out-This is very similar to the behavior form. This form will help keep the student accountable throughout the day to stay on task. The teacher can remind the student of the form when they are working on assignment. If the student completes the assignment then they can receive a sticker on their form. This form will go home with the student every night and be signed by the parents and brought back to school the next day.
Individual and visual schedule-Have the schedule of the days activities on the students desk where they can see it. If they are off task have them look at the schedule to remind them of what they need to be doing. Let the student mark off each task when they have completed it. Send home the next days schedule so they parents can help their child be ready for the next school day. It would also be nice to have a check list of what the students needs to do when it comes to unpacking or packing their things up. This may help them stay focused.
Daily behavior form-First this form will be explained to the student and will be reminded of it every morning at the beginning of the day. The form will be filled out at school and then the student will take it home everyday for the parents to look at. The student will be rewarded for being on task.
Tier 3 Interventions
Organizational Tools-Help the student learn appropriate ways to keep their cubby organized. Create a checklist of what to do when unpacking in the morning and when packing up at the end of the day. Make the check list visual so they understand exactly what they need to be doing.
Mentor-Have the teaching assistant in the class become a mentor for the student. When they have completed a task they need to go to the mentor to have it checked off. This will help keep the student accountable and hopeful help them to stay on task and not get distracted by other things.
The praise game-This is a game that is played by the whole class. The teacher will have 3 key classroom expectations and every 3 to 5 minutes a timer will go off and the teacher will reward students who are demonstrating good behavior. Their team will get a point and at the end of the game they will receive a prize.
Hyperactivity
Tier 1 interventions
If the student is not staying on task then let them know that if they do not finish they will have to miss out on recess to finish their work.
Since this student has a hard time sitting still I would allow them to have something they could fidget with during classroom instruction time.
Frequent breaks-Have the student take frequent breaks in class. If there is an activity longer than 30 minutes have the student take a 5 minute break.
If the student has an outburst or tantrum make sure there are consequences to that type of behavior. There could be a loss of unstructured play time.
Tier 2 Interventions

Reward System-Fist find out what type of reward will best motivate the student. Praise the student when they are performing well and staying on task. They will earn the reward when they are showing good behavior. For example when the teacher sees good behavior then the student will receive a sticker. After 10 stickers are received then the student can pick a prize from the treasure box.

Tier 3 Interventions
Behavior Meetings-At this point the parents should be involved. The teacher and parent can sit down together to discuss the student and what can best be done at school and at home to help the student make better choices. If the school has the resources then have the school counselor join in on the meeting. If there is not support like that at the school then an outside counselor can be suggested to the parents.
Create a Behavior Intervention Plan-Fist take a behavior assessment to know exactly what type of behavior problem the child is dealing with. Then come up with a plan of how to address certain behaviors. For example when the child is throwing a tantrum take the child out of the classroom where they can be in a relaxing environment to help calm down.
Breaks-Give the student a laminated card with the word break on it that they can use when they need to take a break. Give the student a 5 minute timer and a place where they can go and have a short break.
