- - - - avoid sneezing,coughing,heavy lifting and straining when passing stool
      - avoid stooping.use sleep-on shoes to avoid bending to put shoes on.
      - avoid pressure on the affected eye,lie on the unaffected eye.
      - ensure bed rest in the first 24 hours postopperatively unless indicated otherwise
      - admister systemic and local antibiotics and analgesic as prescribed.
      - patient should report no pain and normal vission.
      - maintain eye hygiene
      - no pain is expected.
- - - - provide information about the presentting eye condition and its possible prognosis.
      - instruct the patient and family about how to instill medication
      - and importance of hand hygein when attending to the eye.
      - evaluation
        
        patient demonstrate confidence and an ability to participate in their own care in daily care activities.
        
        patient talks knowledgable about their condition and posible outcomes.
- - - - gain in independence in cases of blindness.
      - nutritional status satisfactory
      - normal vision restored
    - - do eye swabing and administer medication as prescribed.
      - sheild the eyes from the light accordingly and rest the eyes as required.
      - support patients with permanent loss of sight and prepare and guid them on how to intergrate back to the community.
      - give a balanced diet rich in vitamins[A and C] and proteins.
      - refer the patiennt to the blind sociiety for enrollment to programmes so that the patient benefits from them eg get access to guide dogs,bells and sensors.
    - - patient is eating vegetables in the form of salads and fruits
      - legumes and meat for protein to improve eye sightwhere poor sight is a results of poor nutrition.
      - patient report normal vision and demonstrate complete independance.
- - - - ANTERIOR CHAMBER: note the depthpresence of hyphaema or hypopyon
      - PUPILS:note colour regularity, size,shape and reaction to light.
      - CORNIA; note the size, shape clarity and tracks and scars made by foreign bodies or diseased conjuctiva.
      - IRISnote the presence of the synechiae, rubeosis and iridodenesis
      - SCLERA: note colour[yellow in jaundice patients],any nodules of episcleritis, circumcorneal infection.
      - LENS: note clearness of the lens or if you see any opacity.
      - CONJUCTIVA: observe for injection,chemosis,forein bodies and discharge.
      - OCULAR MOVEMENTS: observe for defective movements such as squint and nystagmus.
      - EYELIDS:observe the position, colour, discharge,eodema, scaring and any other abnomalities.
