Pain
Tenderness(Local tenderness)
Rebound pain(when pressure is released)
Rovsing's sign
pathophysiology
Appendix becomes inflamed which leads to obstruction as a result of being occluded by a foreign body. Inflammation process increases intraluminal pressure initiating pain ,pain become localised and appendix fills with pus eventually.
Diagnostic findings
CBC Count
X-ray films
Ultrasounds
Laparoscopy
C-reactive protein
Medication
Administration of IV fluids to correct fluid and electrolyte imbalance and dehydration, IV fluids should be administered prior to surgery.
Administration of antibiotics as per doctors prescription to prevent sepsis.
Drainage , an abscess may form and patient is initially treated with antibiotics
Nursing Assessment
The nurse should assess the level of pain
Laboratory findings should be assessed correctly
assessment of the patients vital signs in preparation for surgery
Nursing intervention
IV infusion is made to replace fluid loss and promote adequate renal functioning.
Antibiotic therapy is given to prevent sepsis.
The nurse must place the patient on a High-fowler’s position to reduce the tension on the incision thereby reducing pain.
Oral fluids could be administered when tolerated
Evaluation of nursing intervention
Pain should be relieved
Prevented fluid volume deficit.
Anxiety should be reduced
Skin integrity should be maintained
Attained optimal nutrition.
