Gram + Cocci
Staphylococci
-
Coagulase test
- differentiates S. Aureus (+ve) from coagulase neg staph
S. aureus colonize 30-50% of human population (on skin and mucosa)
- if altered immune function or breaks in skin/mucosa -> invasive disease can develop
-
S. Aureus
Impt
Leading pathogen in multiple types of infx
- bacteremia
- endocarditis
- osteoarticular infx
- SSTI
- pneumonia
- implanted hardware
-
-
-
-
Diagnosis
Gram stain (+ve), grows readily in standard culture medium, PCR used increasingly for RADT and identification of antibiotic resistance
Treatment
-
Establish extent of infection, Drain pus, remove infected hardware if possible
Prevention and control
antibiotic stewardship, remove or avoid unnecessary prosthetic material, hand hygience
-
-
Streptococci
-
-
Hemolysis Pattern
Beta-hemolytic
-
S. Pyogenes
- Group A
- Bacitracin sensitive (Antibiotic)
S. Agalactiae
- Group B
- Bacitracin resistant
Alpha-hemolytic
-
-
S. Pneumoniae
- Optochin (antibiotic) sensitive
- Capsule
- Quellung (+) (antibody soln, appear to swell)
Viridans
- Optochin resistant
- no capsule
Quellung (-)
Gamma-hemolytic
-
Enterococcus
- Bile esculin +ve
- 6.5% NaCl growth
Non-Enterococcus
- Group D
- Bile esculin +ve
- 6.5% NaCl no growth
-
