Pathophysiology
Diabetic neuropathy occurs when there is an imbalance between nerve fiber damage and repair. The nerve damaging process preferentially affects autonomic and distal sensory fibers, leading to the progression loss of sensation. Metabolic factors seem to prevail in length-dependent diabetic neuropathy whereas inflammation superimposed on ischemic nerve lesion is found in severe forms of focal neuropathies. The thickening and hyalinization of the walls of small blood vessels due to the reduplication of the basal lamina around endothelial cells suggests a role for nerve ischemia in a diabetic neuropathy.
Surgical management
Surgical decompression or decompressive surgery : nerves may be damaged by pressure in certain areas of the legs and arms .Therefore, Doctors are relieving pressure with surgery. The procedure aims to restore feeling to the abnormal parts of the body.
Health Education

Eat food rich in vitamin B12 for example poultry, fish, lean meat ,eggs and fortified cereals
Nursing management
Assess the signs of hyperglycaemia which may result from an inadequate amount of insulin to glucose.
Nursing Care Plan
Assess feet for temperature, pulses, colour and sensation to monitor peripheral perfusion and neuropathy.

Assess for anxiety, tremors and slurring of speech. Treat hypoglycaemia with 50% dextrose.
