antibiotic if severe (amoxicillin, clarithromycin, co-amoxiclav)

rhinovirus, influenza, parainfluenza
coughing, sneezing, nasal discharge, headache
bacterial

high fever, facial pain, headache, nasal discharge (often purulent + watery)
fungal
aspergillus, candida, mucor

invasive (look for confusion, esp in hospitalised/immunocompromised patients)

10 days on penicillin
will only shorten symptoms by 8hrs, but prevents onset of sequelae (e.g. rheumatic fever)
