Jake: self-labeling as a failure
Tier 1 Interventions
sent student on errand to make him feel helpful
ask the student simple questions to begin with that you know he will get right - increases confidence
call parent or email to inform of problem
do unfinished work during recess or lunch
praise when on task or having a good attitude
implement rewards for completing his work

Jake hasn't been doing well in his classes and feels as if he is failing in every way. He even says all the time "I am stupid." He has given up trying on assignments or studying on tests and often pulls other students off task as well.
Data Tracking
Tier 1: form that tracks the data of his participation in homework.
Description: maybe seeing his own participation on homework will make him want to get another "check mark in the box"
Tier 2: Student Learning Style Survey and Student Self Assessment
Let's see how he feels about his own learning style to learn of some new things we may try in class to increase his participation
Tier 3: School Counselor Referral Form & Parent Questionnare Form
The counselor may be able to hone in on any learning disabilities or reasons as to why he fails as if he is failing and does not want to try. The parents may give info on if this is how he acts at home or if it is solely in the classroom
