ONCOLOGY NURSING CARE PLAN
Fear and anxiety due diagnosis of cancer
interview patient and family to assess normal coping style
provide the patient with information about the diagnosis in a supportive manner
allow the patient to express their concerns and fears to provide acceptance of the diagnosi
involve the patient and family in care and facilitate communication
provide care and support to improve the quality of life
refer the patient appropriately
Pain due to cancer and cancer therapy
assess the level of pain using suitable pain method
assess and correct other factors contributing to pain
provide information about pathophysiology of the diseases in a manner that can be understood by the patient
administer analgesics on daily basis as prescribed
teach the patient complementary pain management techniques to augment the pain management regimen
continuously evaluate the effectiveness of analgesics
give appropriate health education
Fatigue and weakness
prepare the patient for possibility of feeling tired
assess the patient's level of fatigue to establish a baseline
assess the patient's ability to perform normal daily activities , to assess the level of dependency
encourage the patient to monitor and document the pattern of fatigue and activities associated with fatigue
control other symptoms that may exacerbate fatigue such as anorexia
monitor the impact of fatigue on patient qua of life to balance activity with the stages of disease and support accordingly
assess the effect of fatigue on the other body functions
educate the patient regarding the interventions to reduce fatigue
encourage the patient to exercise as tolerated to prevent complications of immobility
encourage adequate nutrition to assist in maintaining energy levels
Increased body temperature , denoting infection
monitor vital signs 4 hourly and report any abnormalities
follow up white blood cell count result
administer antibiotics as prescribed to treat infection
initiate measure to minimise infection
apply principles of protective isolation to protect the leukopenic patient
avoid trauma to the skin and mucous membrane
if invasive procedure are unavoidable, assess insertion site daily for evidence of infection
Poor appetite, inadequate intake of food and fluids
provide good nutrition to build resistance
educate the patient regarding the relationship between cancer therapy and nausea and vomiting promote understanding
maintain good oral hygiene to reduce unpleasant taste and stimulate appetite
maintain a pleasant and relaxing environment
ensure a well balanced diet and plan the diet according to the phase of treatment
serve small, bland meals in an attractive manner to stimulate appetite
encourage the use of nutritional supplements
monitor intake and output
maintain pain relief measures
advice the family on nutrition and fluids
Altered family dynamics due to diagnosis of cancer
assess family structure and support system to determine the amount and type of assistance that will be required
provide opportunities for the patient to express their feelings
assist family members to have realistic expectations about the treatment
provide full information about the disease, it's treatment and progress
Painful gums and mouth
assess the oral cavity for lesions
encourage strict oral hygiene, preferably 4 hourly and after meal
soft tooth brush should be encouraged to avoid trauma to the mouth and gums
apply topical anaesthetics and give systemic analgesic as prescribed
dentures should be removed at night
patient should have soft, bland diet and avoid hot spicy food
encourage fluid intake
use Vaseline to avoid cracked lips
educate the family regarding the importance of oral hygine
Ineffective coping
assess the patient feelings regarding their changed appearance
encourage the patient verbalise feelings concerns
provide counselling and psychological support for the side effects of treatment
advice the patient regarding cosmetic issue
avoid use of hair dryers to minimise hair loss
encourage and assist the patient to choose clothes that fit well
encourage the patient to verbalise feelings around sex so that anxieties can be allayed
involve the family in the support and care of patient
Poor compliance with treatment
assess the knowledge and skills of patient to manage cancer care
teach patient and family the required skills
follow up and evaluate these skills at intervals to reinforce teaching
