causes of pure RHF
parenchymal lung disease (cor pulmonale)
diseases of pul vasculature
primary pul hypertension (idiopathic, arterial, rare, vasoconstriction)
recurrent PEs
RHF symptoms
high pressure in IVC -> back pressure
ascites (oedema of the bowel wall)
hepatosplenoegaly
portal hypertension -> congestive splenomegaly
congestive hepatopathy
nutmeg liver (speckled pathological appearance)
congestion around central vein (dilated sinusoids)
v rarely causes cirrhosis (called cardiac cirrhosis/sclerosis)
fibrosis
if severe + longstanding
raised JVP
peripheral pedal LL subcut oedema (common)
pleural effusions
right atrial dilatation
right ventricular dilatation +/- hypertrophy
pericardial effusions
initial insult
myocardial injury
