Identify and Help Struggling Student
Phase 1. Recognition
Student exhibits atypical needs as compared to peers
call a meeting with the parents or guardians
Provide examples of the student’s work and/or anecdotal classroom notes regarding the student’s needs
Phase 2. Pre-Referral
In case of Successful intervention, the Process stops.
If the Interventions are not successful then the process move to Referral
Phase 3: Referral
Student is then officially referred for evaluation for special education services by an adult with intimate knowledge of the student’s ability.
Phase 4: Evaluation
With consent of the parents, the student is evaluated with a variety of assessment tools and strategies. The assessments must not discriminate and should provide information to help determine the unique needs of the student.
Phase 5: Eligibility
IEP team determines if the student is eligible for special education services. The student must have a disability that negatively impacts his/her educational performance and the child needs special education services in order to benefit from education.

Phase 6: IEP Process (IEP and LRE)
A multi-disciplinary team of parents, general and special education teachers, administrators, and others meets to develop the Individualized Education Program (IEP). This document guides the special education program that will be provided for the student. The IEP team must also determine the least restrictive environment (LRE) and provide justification in the IEP for more restrictive placement.

Helping Struggling Students in my classroom
Here are some of the methods I used in my classroom to help my struggling students.
Build Relationship
I found building relationship with the struggling students as the most helpful tool. If they are not comfortable with me then they won't listen to me. When they are comfortable they are open to learn.
Collaboration
Collaboration is the key in any classroom. I made sure that the child get ample opportunities for peer learning.
Differentiated instruction
I explained to the kids that they are not competing with each other but they are learning and progressing as individuals. So all of them won't always have dame work. I differentiate by content, by skill and by abilities.
Differentiated instruction strategies:
Flexible seating
Flexible seating is not only providing beautiful seating options but to allow the students to make choices as long as they are safe. I allow them to choose their spots - floor, carpet, bean bags, wobble seats, chairs and so on.
Extra time
I provide students extra time if they need to finish and assigment. They are able to retake test as many time as they want.

We don't have any specific referral process in our school. The other schools in my district follows the referral plan stated below.
