Intro to fungi (i)
Intro
-
worldwide, but certain types endemic in specific regions
-
-
-
cell membrane = phospholipid bilayer containing high levels of ergosterol (binding site for some antifungals)
-
-
-
-
-
-
need special medium: Sabouraud dextrose agar - contains peptone, acid + antibiotics to suppress bacterial growth (hence selective)
Classification
-
-
moulds/filamentous fungi
e.g. aspergillus, dermatophytes
-
produce branching intertwined hyphae (tubules) + spores aka conidia (reproducing bodies, germinate + form hyphae)
-
-
-
Pathogenesis
-
-
invasion can be superficial (dermatophytosis, thrush), subcut (sporotrichosis), or invasive (cryptococcal meningitis)
hypersensitivity to fungal antigens can occur (aspergillum-incidce asthma, broncho-pul aspergillosis)
-
Intro to aspergillus
in soil, air plants, dust (e.g. @ construction sites - spores inhaled, food, decomposing organic matter)
NB if there's construction in the hospital it must be preapproved, + CF + immunosuppressed patients should avoid this area
angioinvasive (destroys blood vessels to disseminate, causing haemorrhaging)
-
clinical manifestations
allergic aspergillosis
-
-
-
-
bronchospasm, obstruction, wheeze, cough, dyspnoea, fever
