Mental Status Assessment
Appearance
Body Movements: Movements of the body are voluntary, coordinated, smooth, even, and deliberate.
Dress: The patient is dressed appropriately for the season and setting, as well as for their age, gender, and social group. The patient's clothing fits well and is worn appropriately.

Mini-Mental Exam

Naming
Have the patient tell you what different objects in the room or on you are called: clock, watch, pen, pencil, clipboard, light switch.
Memory
Tell the patient the name of three objects and have the patient repeat them back. Then have the patient recall the three objects later in the exam.

Writing
Have the patient write a made up sentence. Evaluate parts of speech, spelling, and cohesion.

Cognition
Attention Span: The patient is able to concentrate and articulate a full thought. The patient is able to follow directions.
Recent/Remote Memory: The patient is able to recall information and events from the past 24 hours. The patient is able to recall historical events and significant dates which are specific to them.
Orientation: The patient is oriented to person, place, and time.

Behavior
Facial Expression: The patient's facial expression is appropriately and changes according to the situation at hand. Eye contact is appropriate.
Speech: The patient's speech is smooth, effortless, articulated, and of appropriate volume.
Level of Consciousness: The patient is alert, aware, and awake. The patient responds reasonably and appropriately in a timely manner.
Mood and Affect: The patient's body language, mood, and facial expression is appropriate and changing with the person's place and condition.
