in animals causes chronic renal infection -> polyuria -> contaminates water -> transmission to humans

disseminates from inoculation site into bloodstream causing systemic vasculitis, endothelial injury, blood vessel cell damage, necrosis

Clinical manifestations
-
90% have mild ILI, 10% are severe
biphasic course
initial spirochaetemia (fevere, headache, myalgia, conjunctivitis) either resolves after 5-7 days or progresses to immune phase (aseptic meningitis or Weil's)
Lab Dx

PCR only in reference labs of urine with in 1st wk, or urine if longer

avoid water sources, esp if skin has cuts/abrasions
