Leadership and Management in Nursing Practice
Contemporary Theories of Managment
Democratic Leadership:
Encourages open communication so that staff may participate in decision making (Bennet, 2009). It is universally accepted as the basis for nursing leadership (Smith & Johnson, 2018).
Effects on Patient Safety and Outcomes
This method promotes improved patient outcomes and safety (Medley and Larochelle, 1995).
Transactional Leadership:
The leader promotes compliance from staff through both rewards and punishment (Boamah & Tremblay, 2018)
Effects on Patient Safety and Outcomes
This method attributes to the emotional exhaustion of staff and nursing burnout, therefore it may cause decline in patient safety and outcomes (Boamah & Tremblay, 2018).
Transformational Leadership:
The leader models exemplary behaviors to inspire staff (NMBA, 2016). This leads staff to transform and improve for the good of the unit as a whole (Johnson & Smith, 2018; Aiken et al., 2012).
Effects on Patient Safety and Outcomes
Transformational leadership
is associated with increased patient satisfaction. In 4 of 5 studies, transformational leadership showed to reduce medication errors. The transformational leadership style is also was associated with lower mortality rates in 3 of 6 studies (Aiken et al., 2012).
Additional findings show nurses are in favour of the transformational approach as it promotes intrinsic motivation and job satisfaction. We know that nursing satisfaction is directly related to improved patient outcomes and safety (Fast & Rankin, 2018; Aiken et al., 2012).
Authoritarian Leadership
: A leader making all decisions with little input from staff (Bennet, 2009)
Effects on Patient Safety and Outcomes
Authoritarian Leadership promotes a culture of blame and leads to increased rates of adverse safety events and poor patient outcomes (Bennet, 2009).
Laissez-faire Leadership:
A "Hands off" approach where the leader provides little to no direction or supervision (Ignatavicius, Workman & Rebar, 2018).
Effects on Patient Safety and Outcomes
Laissez-faire Leadership
is associated with high nurse turnover and creates a culture of blame which are both directly related to poor patient outcomes and an increase in adverse safety events (Aiken et al., 2012).
Impact of Health Policy on Nursing Managment
The Nursing and Midwifery Board of Australia (NMBA), describes nurse managers as change agents. They work with staff and other management individuals to implement new policies and procedures on the unit level (Ignatavicius, Workman & Rebar, 2018; NMBA, 2016)
The politics of health care and the policy-making process shape nursing leadership. They affect what decisions nurses in leadership positions can make (Ignatavicius, Workman & Rebar, 2018; Aiken et al., 2012)
Managers who have policy competence improve patient outcomes and nurse satisfaction by directing their organisations to make policies that have desirable effects on their organisations (Sagar, 2015)
