Unfocused/Inattentive
Tier 1 Intervetions
Engage student- I will make additional effort to connect with Phong in class and out of class to find common ground. Building a stronger rapport with Phong might help keep him on task and focused in class.
Call on student frequently. I normally use a random name generator in class but I will also make an additional effort to call on Phong more frequently to keep him focused.
Tier 1 Data Tracking
Simple Behaviour Chart. At this stage Phong will be given a simple behaviour chart and the teacher will keep a copy of one as well. The chart will simply record how the lesson is done by filling in an emoji. At the end of each week, I will make time to talk to Phong and compare our behaviour sheets.

Tier 2 Interventions.
Daily Behaviour Form. If the problems with Phong persist then I will create a daily behaviour form. I will set it up to monitor his performance every lesson for 3 weeks and have it sent home to be signed at the end of each week so his parents can see his progress. If at the end of 3 weeks he will has shown enough improvement he will be given a reward and assigned 1 more behaviour sheet. If at the end of the second 3 week period he has shown satisfactory improvement then he will be given another reward and will no longer have daily behaviour forms.
Mentoring. Phong will also be assigned a mentor. The mentor would ideally meet Phong twice a week and make an effort to connect with him and get to the root of the problems he is having in class.
Tier 2 Data Tracking

I will create a live Microsoft Excel document where all teachers who have Phong on a behaviour form can upload his results so that we can all monitor his progression collectiveky.
Tier 3 Interventions
Behaviour Contract. If the problems with Phong continue then I and any other teachers who are having a problem with him will draw up and have him and his parents sign a behaviour contract.
Behaviour meetings. To help monitor this we will also need to have regular behaviour meetings with Phong and his parents if possible.
