(8) Recommendations

its ok to have a statement that further assessment is required, but be specific
Typically each recommendation is numbered and briefly highlights how that recommendation addresses a finding in the report
(1) Identify Information

Personal idetails , bullet point, name, age date of birth, date of assessment, address agenda, year at school, martial status.
Use of interpreters, if used this should be stated

Just present the results, start broad (full IQ) and work down to the particular areas eg. sub scales,

(7) Interpretation
This is where focus on referral question, and pull everything together, - background info, test info, obs made during testing
describe limitations, and how may influenced testing
Make clinical judgement - back up with examples, eg distractable, based on observations and linked back to referral question.
(3) Test Administered
Tests administered, listed separately, with adequate rationale for their use? This will help the "uninitiated" understand the purpose of the test. eg WISC
Should test scores be included in this report and for recipient? Depends on purpose of the report, state the IQ (another psychologist) or the average range (non psychologist). (For assignment use score and confidential intervals)
(2) Reason for referral
Who made the referral? Purpose of report? what Q's were asked of you? when the person sent to you? Who is the report to be sent too? State, name, title and organisation
