Disclosure by victim
Affirm that you believe them, it isnt their fault, right to be safe
Reassure that the discussion is confidential (except in exceptional circumstances)
Assess the patient (physical, mental)
Refer to other services (maternity, sexual assault, substance miscuse)
Contact the designated person for DV to assess if person/children in immediate danger (SafeLives DASH Risk Checklist, RCGP Safeguarding Children Toolkit)
If immediate danger, call police, initiate adult and child safeguarding, ideally with consent
If no immediate risk, refer to designated DV person for assessment, advice, referral
If patient refuses, provide information on support groups and that you are here
when they are ready to take action
Record in detail and accurately all discussions and actions
