Small Bowel
Vomiting occurs early, e.g. after meals

Mechanical Obstruction

Bowel above the level of obstruction is dilated, with increased secretion of fluid into the lumen
Functional Obstruction

Signs: bowel sounds are absent, pain is less
Paralytic Ileus
Contributing factors: surgery, pancreatitis, spinal injury, hypokalaemia, hyponatraemia, uraemia, peritoneal sepsis

Pseudo-Obstruction
Predisposing factors: pelvic surgery, trauma

Pathology
Closed Loop: obstruction at two points, forming a loop of grossly distended bowel at risk of perforation e.g. sigmoid volvulus
Strangulated: Blood supply is compromised and the patient is iller than you would expect. There is sharper, more constant and localized pain
Simple obstructed bowel: one obstructing point, and no vascular compromise
Volvulus
Sigmoid Volvulus

Occurs in elderly, constipated patient
Produces severe, rapid, strangulated obstruction
Treatment

If not treated, perforation can occur leading to fatal peritonitis

Leads to ischaemia, necrosis and perforation of loop of bowel
Stomach Volvulus
Classic triad: vomiting, pain, failed attempts to pass NG tube
