Certification
Dr certification (Statement of Fitness for Work)
GP or hospital doctor completes for incapacity >7d
During first 6m can only be issued for max 3m
Can state that patient is unfit for work or may be fit to
work under certain circumstances e.g. reduced hours
Can record patient's functional limitations
Can be issued on the day you assess the patient
or after assessment if input from another HCP needed

Self certification
Individual must self-certify for first 7d
Different forms for those who can claim statutory sick pay
and those claiming universal credit
Work Capacity Assessment
Carried out by the employment advisors for
the Department of Work and Pensions (DWP)
Assesses variety of physical and mental characteristics to assess fitness for work
Performed <13w of any claim
Needed from start of incapacity for ESA or UC
Needed from 28w incapacity for SSP
