Injury : Right Elbow, Neck, Lower Back and Buttock
The Employee has notified the ministry that he was injured in a work-related accident in the course of his employment on DOA at the said accident site.

On the above mentioned date, time and place, I fell from a 3 meter height onto the metal bars. I was brought to Farrer Park Hospital by my employer on the same day.
3 Under the law, employers are required to report work-related accidents to the Ministry. However, till date we do not have any records that you have reported this accident to the Ministry. We take a serious view of this omission.
4 You are now given an opportunity to explain in writing why legal action should not be taken against you for failure to report the accident. Should you disagree that this accident is work-related, please provide the necessary supporting evidence.

h) Why did the company not submit the iReport to MOM? If the IW has more than 3 consecutive days of MC, please submit the accident report (iReport) as soon as possible to MOM through online - www.mom.gov.sg/ireport
6 You are to provide your reasons in Para 4 and the documents in Para 5 to us by 12/02/19 (5 working days from this email) , failing which the Ministry will deem the company agree to the accident (happen out of and in the course of employment), and admit the claim under Section 3(6) of the Work Injury Compensation Act. We may take enforcement actions against you for failing to report the accident to MOM.
