Risk Screening mandated by Standard 5
Pressure Injuries (PI) and Wound Management
Conduct comprehensive skin inspections
Screened or assessed using a validate tool within 8 hrs of presentation
Reassess
Complete notification for each PI in local incident reporting system (e.g. IIMS), and to appropriate medical team
For pts at risk (with or without PI)
Implement prevention strategies within 2 hours of assessment
Review strategies 4-hourly, at handover, on transfer at every intervention
PI Assessment
No screening required if using a validated tool (nonsense?)
Required when PI identified or on transfer of care at next dressing change
2 parts
Use a validate PI Risk assessment tool/process appropriate for patient population
Skin assessment based on visual inspection
minimum daily for inpatient at risk of developing PI (2 part assessment)
minimum weekly for inpatient not at risk or low risk (2 part assessment)
at each care intervention or positioning change for inpatient with PI (skin inspection and pain assessment)
Wound Reassessment
at least weekly
Wound mgt to be reviewed if not healing at optimal rate (25% reduction in 4 weeks)
Pain assessment (for pt with PI)
at least every shift using a validated tool
