I am a on my Fieldwork IIA, picking up patients for my CI during week 9. I discovered that there is no evaluation in the chart and no notes have been written for 2 months. My supervisor has asked me to “fill in the documentation” so the facility can bill for these treatments. I don’t even know if the treatments were done but this doesn’t seem right to me. What should I do?
**Responsibility Issue:
OT is responsible for completing her documentation in a timely manner. If the OT has difficulty performing her obligation, she needs to reach out to her supervisor and see the help she needs.
Decision
Talk to OT directly about speaking ther supervisor on her overdue documentations and come across a solution for her to resolve this matter without involving the student.
I would take the next step to talk to the Rehab director. Rehab director will have the take action on the consequences of incomplete documentation and making sure to check on her daily documentation performances to make sure it's completed on time.
Ethical Issue: Justice
Principle 4: Justice is voilated because OT did not promote fairness and objectivity in the provision of
occupational therapy services. This includes the documentation part of therapy services provided.

Ethical Issue: Veracity
Principle 5: Veracity is voilated because the CI is not truthfully compeleting her documentation based on the treatment sessions she has conducted within the last two months. Asking a student to complete the sessions, he or she did not part take in is fraud.
In all situations, occupational
therapy personnel must provide accurate information in oral, written, and electronic forms. CI is not compliant to this.
Decision :
speak to the OT about her needing to completing these before moving forward and taking more case loads.

Responsibility issue:
Student is being placed in a situation where she will also will be breaking the code of ethics. Part taking in OTs unethical way will lead the student to jeopardize her learning experience.
