Spontaneous Pneumothorax
Signs/Symptoms - General
tachypnea
diaphoresis
cyanosis
can be seen late + in large/tension pneumothorax
SpO2 <94%
Facts
sudden rupture of portion of visceral lining
not caused by trauma
causes lung to partially collapse
Pathophysiology
Primary Spontaneous Pneumothorax
in patients w no underlying lung disease
occurs in teen-early 20s who are tall and thin
from activity such as coughing, straining, or lifting heavy object
Secondary Spontaneous Pneumothorax
in patients w underlying lung disease
many have history of smoking or connective tissue disorder
occurs in pts w history of lung disease
occurs in pts who are more prone from areas of weakened lung tissue called blebs or bullae
allows air to enter pleural cavity
disrupts notmal negative pressure
causes lung to collapse
Treatment
PPV + supplemental O2 if needed
Signs/Symptoms - Exclusive
sudden onset of shortness of breath
sudden onset of chest or shoulder pain
subcutaneous emphysema
pallor
decreased breath sounds to one side of chest
most often heart first at apex
