the disease is caused by exotoxin secreted via macrophage at the site of infection .. it is cleaved into two proteins polypeptide A -terminate host cells-B-binding- the receptors for toxin molecules is heparin binding epidermal growth factor present on cardiac and nerve cells ,, Eleck plate toxigenicity test is used.
Exudative pharyngitis : form thick adherent psedumembrane which may cause respiratory obstruction , low grade fever and toxin this is absorbed and cause damage to distant organs - heart , NS , kidneys and adrenal- , transported via droplet
tonsillitis : the patient may develop bull neck oedema and tenderness , severity of complication related to the extent of local infection
diagnosis : clinical severe exudative tonsillitis vaccination history , travel history , Nasal , throat and pharyngeal swab culture on selective media -cystine tellurite gar- , PCR
