After consistent, research based interventions have been provided & no evidence of progress is noted, then a referral for SP ED testing my be suggested.

also the student performing at a level that is well below grade level expectations
n differentiating it should be apparent that all students work at varying abilities and there for students should be granite and opportunity to work at a pace that meets their deficits without comparing them to their peers
Parents should be made aware of the specific deficit areas as well as given adequate information on how to assist a student when they are at home.
It is also important that information like this is documented especially if you're dealing with students that have individual education plans or IEPs
